Medical records

AHN patients have the right to access their own health information. There’s no fee to release medical records directly to other doctors or health care facilities.  There are three simple ways to request your medical records:

  • Request digitally through MyChart: If you have a MyChart account, just select “Medical Record Request” from the main menu and send your request electronically — so you get your records even faster.
  • Complete an authorization form: No MyChart account? No problem. Just fill out a form to request your AHN medical records or transfer records from a previous provider to AHN. Go to medical records for more information.
  • Call your doctor, hospital, or medical facility: If you have questions about requesting information, or you need help filling out forms, reach out to your specific doctor or the facility. You can find phone numbers for each of our facilities in our locations directory.

AHN Billing

We do our best to make our bills easy to understand, but they can still be complex. If you have questions or need financial assistance, we’ve got two ways for you to get help:

  • Call our billing department: Call (844) 801-8400 and select option 1 for billing support. We’re available weekdays from 9:00 AM to 5:00 PM.
  • We’ll call you: Use our contact form and we’ll call you back.

You can pay your bill online anytime using MyChart.
